Due to MWR’s commitment to continually improving the experience of our customers, reducing paper waste, and creating a more sustainable office, we are working to create a 100% e-form processes. All of the forms used by MWR are available on this page. To complete and submit a form to MWR please follow these steps.

  1. Download the form you need (should download just by clicking on the link)
  2. Open the downloaded form in Adobe Acrobat Reader®.
  3. Fill out the form including the e-sign field (when available) and select the “Submit” button.
  4. This will open an email client with the “To” and “Subject” fields pre-filled and the completed PDF attached.
  5. Send email.

We are still accepting all forms in person either to the RBH or the satellite office, via scan and email.
